Welcome to My Venus Club. Our services are delivered through telehealth, allowing you to connect with our clinical team remotely. Please review the following carefully before participating.
1. What is Telehealth? Telehealth involves the use of electronic communications — including video conferencing, messaging, and remote monitoring tools — to provide healthcare services without an in-person visit.
2. Nature of Services My Venus Club provides personalized wellness and health optimization services under the supervision of licensed healthcare professionals. Services may include health assessments and consultations, review of diagnostic testing and wearable data, nutrition and lifestyle recommendations, ongoing coaching and support, and consideration of treatment options where appropriate. A provider-patient relationship is established only after a formal evaluation and acceptance into care where permitted by law.
3. Potential Benefits Telehealth may provide convenient access to care from your home, more consistent communication and follow-up, personalized data-driven health insights, and reduced need for travel or in-person visits.
4. Potential Risks & Limitations Telehealth has limitations, including technical issues such as internet or device failure, inability to perform an in-person physical examination, possible delays in diagnosis or treatment, and reduced ability to respond to emergencies. You understand that telehealth may not be appropriate for all conditions.
5. Privacy & Security My Venus Club uses HIPAA-compliant platforms to protect your personal health information. However, electronic communication carries some inherent privacy risks. You are responsible for using secure devices and internet connections. Your information will be handled in accordance with our Privacy Policy and applicable laws.
6. Prescriptions & Treatment If clinically appropriate, treatment recommendations and prescriptions may be provided only after proper evaluation. Medications are dispensed through licensed U.S. pharmacies. There is no guarantee that any medication will be prescribed.
7. Patient Responsibilities By participating in telehealth services, you agree to provide accurate and complete health information, follow recommended care plans and instructions, ask questions if you do not understand any part of your care, and seek in-person care when advised.
8. Emergency Situations Telehealth services are not intended for emergency care. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.
9. State Licensure Telehealth services are only available to individuals located in states where My Venus Club providers are licensed to practice.
10. Consent to Telehealth By using My Venus Club services, you acknowledge that you have read and understand this Telehealth Consent, you understand the risks and benefits of telehealth, and you voluntarily consent to receive services via telehealth.
